Use validated tools like PHQ‑9 (depression), GAD‑7 (anxiety), K10 (psychological distress), PCL‑5 (PTSD), AUDIT‑C (alcohol), and DAST‑10 (drug use) to quantify symptoms and monitor outcomes over time.
Scores are auto‑scored and mapped to severity bands where applicable (e.g., PHQ‑9 minimal → severe), supporting quick risk triage and treatment calibration.
Your Outcomes widgets visualize time series, latest score deltas, and suggested cadence (e.g., every 14 days for PHQ‑9/GAD‑7).
Assigning Assessments
Open Assign Homework → Assessments, select a measure (e.g., PHQ‑9), pick a due date or set a recurrence (daily/weekly/bi‑weekly/monthly), then click Assign.
Assessments appear under the related plan strategy with “Pending Approval” until you approve and send the plan.
Previously assigned measures are greyed‑out to avoid duplicates. New assessments you enable in your catalog will automatically appear in the picker.
Reviewing Results
Open the client’s Explore → Outcomes widget to view trendlines, severity bands, latest score and change since prior administration.
The system computes a next‑due date based on each instrument’s typical cadence so you can maintain consistent follow‑ups.
Clinical Use
Pair assessments with goals and activities (e.g., CBT or Habit Building) to evaluate intervention impact over time.
Document interpretation and next steps in your notes; these instruments support—not replace—clinical judgment.
